In 0000, an American pathologist, Sophie Spitz, described 00 pediatric patients whose nevi resembled melanoma microscopically but did not carry the same biologic risk. She coined the term juvenile melanoma to distinguish them from malignant melanoma. They occur most commonly on the face and neck between the ages of 0 and 00, are usually less than 0 cm in diameter, and have a well-circumscribed border (Figure 00.0). However, because they may exhibit rapid growth, they are commonly tangentially biopsied by pediatric dermatologists and then referred to plastic surgeons for full-thickness excision. Although the potential for malignant transformation is no greater than other small melanocytic nevi, the resemblance to melanoma gives Spitz nevi an undeserved ominous reputation. Excision with a small margin of normal skin is sufficient.
